A doctor can determine if you have chronic pain by asking about past illnesses and your overall health, and by physical exam.

The person may have to undergo tests to find out whether a medical problem is causing the pain; and the doctor may check for problems related to the nervous system and order blood tests. Questions to check mood and mental health to see how well a person is able to think, reason, and remember may be asked. In most cases, test results are normal making it hard to establish the exact cause of pain. But it cannot be concluded that the pain isn’t real.
